Corticotomies and accelerated orthodontics in patients with thin periodontal biotype: a scoping Review

Abstract

Orthodontic treatment in patients with thin periodontal biotypes poses a clinical challenge due to increased susceptibility to dehiscence, fenestration, and gingival recession during tooth movement, which has led to the use of corticotomies as a therapeutic acceleration strategy. The objective of this scoping review was to map the existing scientific evidence on the use of corticotomies in accelerated orthodontics in patients with thin periodontal biotype. A review was conducted following the PRISMA-ScR guidelines, including human studies published between 2016 and 2025 that evaluated cortical activation techniques and reported periodontal outcomes. Selective alveolar corticotomy, PAOO, piezocision, and micro-osteoperforations were mainly identified. The results show periodontal stability in clinical parameters and potential improvement in bone support when grafts are associated, with mostly transient adverse events. In conclusion, the available evidence suggests that these techniques do not systematically compromise the periodontium; however, limited phenotypic characterization and poor methodological standardization persist.
