Queratitis herpética epitelial y estromal en paciente pediátrico: reporte de caso clínico

Resumen

La queratitis herpética es una infección corneal causada por el virus herpes simple tipo 1 y constituye una de las principales causas de ceguera corneal infecciosa a nivel mundial. En la población pediátrica representa aproximadamente entre el 10 y el 15% de los casos, con riesgo significativo de recurrencia y secuelas visuales permanentes. Se presenta el caso de un paciente masculino de 7 años que consultó por fiebre, dolor ocular, fotofobia y lesiones vesiculares perioculares en el ojo izquierdo. El examen oftalmológico evidenció hiperemia conjuntival, opacidad corneal central compatible con leucoma y lesiones dendríticas características de queratitis herpética con compromiso epitelial y estromal. El manejo incluyó limpieza quirúrgica, tratamiento antiviral tópico y sistémico, y antibioticoterapia profiláctica, con evolución clínica favorable. Este caso destaca la importancia del reconocimiento temprano de los hallazgos clínicos característicos y del manejo multidisciplinario oportuno para prevenir complicaciones graves como adelgazamiento corneal, perforación y pérdida visual permanente.
