Acute Liver Failure as the Initial Presentation of Cystic Fibrosis in an Infant: An Unusual Pediatric Case Report
Abstract
Cystic fibrosis is an autosomal recessive multisystem disorder predominantly affecting the respiratory and pancreatic systems; hepatobiliary involvement occurs in approximately 20% of patients, whereas severe hepatic presentation during infancy is uncommon. We report a five-month-old infant presenting with progressive jaundice, abdominal distension, hepatomegaly, marked hypoalbuminemia, and coagulopathy consistent with acute liver failure. Imaging revealed heterogeneous liver disease and pulmonary involvement without bronchiectasis. The sweat chloride test was positive (68 mmol/L), and whole-exome sequencing identified the pathogenic F508del variant in the homozygous state; this genotype is found in approximately 43% of genotyped patients with cystic fibrosis. Following multidisciplinary management, the patient showed progressive improvement in liver function and respiratory stabilization. This case highlights the importance of considering cystic fibrosis in infants with unexplained acute liver failure and emphasizes the value of sweat chloride testing and timely molecular diagnosis.
